A 70yr old female presented to casuality with complaints of distention of abdomen and shortness of breath grade-3 since 5 days

The patient was apparently asymptomatic 5 days ago then presented with Abdominal distension since 5 days.Distension is generalized.No History of palpitations ,pedal edema,oliguria,fever,cough,fatigue.Hypothyroidism since 5 years.No history of hypertension,diabetes asthma tuberculosis.she was on Thyronorm100mg OD for hypothyroidism.

Investigation ordered:

ECG,

2D-Echo

Diagnosis:

HFrEF with Atrial fibrillation 2 to IHD

Treatment:

1. Inj. Amiodarone 150 mgIV stat (2 doses)
2.Inj.Amiodarone infusion
 1mg/min till 6hr f/b 0.5 mg/min for next 18 hours
3.Inj.clexane 40mg Sc OD

We have entered into 3rd semester 2months ago. We were excited as well as quite nervous that we are going to experience the hospital atmosphere for the 1st time, But due to this pandemic we are unable to appear directly and experience the patient but even though we had a very good experience .Although the online classes were quite turbulent in the starting because of our Professors, slowly everything started falling into place. And even the clinics are going on perfectly because of the efforts of our Professors in helping us understand the cases thoroughly. And finally, I would like to convey my special thanks to the department of general medicine for encouraging us day by day to improve our knowledge.The E logs which has been kept for us are helpful and made me learning so many new things. Once again thank you for this opportunity.
